MANDATORY HEALTH INSURANCE


The Board of Trustees of the California State University (CSU) requires that all international students must have health insurance as a condition of registration and continued enrollment in the California State University system.

Health Insurance Requirements for ALL Students:

  • Major medical coverage for each illness and each injury: minimum $50,000 USD
  • Repatriation: minimum $7,500 USD
  • Evacuation insurance: minimum $10,000 USD
  • Proof of health insurance must be in English and the amounts must be in U.S. Dollars

Insurance from Your Home Country:

If you purchased a health insurance plan in your home country, you must fax proof of your health insurance to SAL office 5 working days before the last check-in date. Our office is unable to contact the insurance companies so please make sure that the proof is in English, shows the amounts in U.S. Dollars, and meets the health insurance requirements above.

Once the proof is reviewed, we will send you an email to let you know if the health insurance is approved or not.

If you are unable to provide the proof 5 working days before the last check-in date, or if your insurance is NOT approved by our office, you will be required to purchase the health insurance plan we offer when you report to our office. Please see below for the rates and coverage dates.

CSU Healthlink/Wells Fargo Student Insurance (in the U.S):

Please note that students must buy the correct coverage term for health insurance from Wells Fargo Insurance Services. SAL will NOT accept the incorrect health insurance coverage and the student will need to contact the health insurance company to fix the incorrect term purchased.

To purchase a health insurance plan in the U.S. you should visit the CSU HealthLink/Wells Fargo Student Insurance Division website:

Summer 2008

1. Click "Student Insurance/College Health Plans"
2. Search by school by typing in "San Jose State University"
3. Click "Enroll" as a "STUDIES IN AMERICAN LANGUAGE Student"

If you are attending SAL for the June 2 to August 13, 2008 term OR June 2 to October 17, 2008 term, please pick the SUMMER health insurance coverage.
Coverage Date for SUMMER: May 2, 2008 to August 13, 2008
Student rate: $238
Spouse: $705
Per Child: $359

* Note: The SUMMER health insurance plan will only cover students until August 13, 2008. Students, who are attending the June 2 to October 17, 2008 term, will need to purchase the additional insurance coverage term to insure them for their entire term. More information will be provided in June 2008.

RETURNING STUDENTS

Returning students must also submit proof of health insurance with their tuition payment. You are required to provide all the necessary insurance documents by the given deadline (announcement of health insurance deadline will be provided to continuing students when the registration period begins).

ADDITIONAL IMPORTANT INFORMATION

Starting fall 2004 all SAL students who purchase Wells Fargo Student Insurance must pay a total of $150 deductible per policy year. A deductible is the amount subtracted from the total amount payable for a Covered Expense before payment of benefit is made. Whenever possible, you are strongly encouraged to use the resources of Student Health Services located on SJSU's campus, but it is not required. Student Health Services can help you locate off-campus medical providers when you need additional care or specialists. In addition, prescriptions obtained at Student Health Services will be reimbursed at 100%.

An SHS referral does not guarantee that the services required will be considered as eligible expenses under the plan, nor is it a guarantee of payment.

HOW DO I FILE A CLAIM?

Students must mail the claim bills directly to BC Life, not through SAL office. Make a copy of the receipts and bills for your own records before mailing them out. It is a good idea to send them by Certified Mail.
