The Board of Trustees of the California State University (CSU) requires that all international students must have health insurance as a condition of registration and continued enrollment in the California State University system.
Health Insurance Requirements for ALL International Students:
- Major medical coverage for each illness and each injury: minimum $50,000 USD
- Repatriation: minimum $7,500 USD
- Evacuation insurance: minimum $10,000 USD
- Proof of health insurance must be in English and the amounts must be in U.S. Dollars
Insurance from Your Home Country:
If you purchased a health insurance plan in your home country, you must fax proof of your health insurance to the SAL office 5 working days before the last check-in date. Our office is unable to contact the insurance companies so please make sure your proof of health insurance is in English, shows the amounts in U.S. Dollars, and meets the health insurance requirements above.
After the proof is reviewed, we will send you an email to let you know if the health insurance is approved.
If you are unable to provide the proof at least 5 working days before the last check-in date, or if your insurance is NOT approved by our office, you will be required to purchase the health insurance plan provided by Wells Fargo Insurance Services when you report to our office. Please see below for the rates and coverage dates.

CONTINUING STUDENTS
Continuing students must also submit proof of health insurance with their tuition payment and provide all the necessary insurance documents by the given deadline. Announcements of the health insurance deadline will be provided to students when the continuing student registration period begins.
STUDENT HEALTH CENTER
Whenever possible, you are strongly encouraged to use the resources of the Student Health Center located on SJSU's campus, but it is not required. The Student Health Center can help you find off-campus medical providers when you need additional care or specialists.

GENERAL INFORMATION ABOUT HEALTH INSURANCE WITH WELLS FARGO INSURANCE SERVICES:
Starting Fall 2004, all SAL students who purchase Wells Fargo Student Insurance must pay a total of $150 deductible per policy year. A deductible is the amount subtracted from the total amount payable for a Covered Expense before payment of benefit is made. Students must mail the claim bills directly to Anthem Blue Cross, not through the SAL office. Remember to photocopy your receipts and bills for your own records before mailing them out. It is a good idea to send them by Certified Mail.
